0215 (3): Motrin, diaper cremes, and sunscreen on site without parent permission or medical authorization forms. 0215 (3)(a)(ii): Diaper Cremes and sunscreen on site without child's first and last name on medication. 0215 (3)(d): Unused Mortin, diaper cremes, sunscreens, other medications on site without authorization forms that were given to provider by the parents. ALL medications were thrown in the garbage can while this Licensor was on site.
